  • This survey must be completed by a person expressing their own attitudes regarding the Team atmosphere in their department.

  • A response must be given for all 110 questions.

The purpose of this questionnaire is to obtain your peceptions of the team that you are or were on. We are interested in gathering informatoin on how persons perceived a SUCCESSFUL TEAM and what may have contributed to its success.

In order to complete the survey think about a successful team you are on now or were on in the past. Think about the type of team it was (i.e. sports, work related, health care, etc.), then, provide a brief description of the team in the box below.
